A UP Diliman faculty member or staff should fill up this form and submit to the Chancellor (through channels) prior to engaging in the private practice of one's profession. The privilege of private practice is granted for a maximum period of one year.
As stated in Chapter 10 of the UPD Faculty Manual (Dec. 2003):
"Permission to engage in private practice of profession may be granted provided
that such practice:
-- is not adverse to the interests of the University;
-- shall not be conducted on official time;
-- will improve the person's efficiency and usefulness to the University; and
-- shall be subject to such other requirements as may be imposed by law or University
rules and regulations."
For renewal of this privilege, the faculty/staff must again fill up this form and submit
